The Federal Heights Police Department is committed to providing the best service possible to our residents. We welcome your feedback, both positive and areas for improvement.

The form below will allow you to submit feedback for review by the Police Department. Please fill out the information below to submit your feedback about the level of service you received or an incident you observed. You are not required to identify yourself in this form; however, we are unable to follow-up or provide additional information to those who do not include contact information.

Once submitted, this form will be delivered to the City Manager, the Police Chief and Senior Police Staff to ensure that your concerns are addressed as completely as possible.  If you would prefer to fill out a downloadable version of this form, it can be found below. Please address a paper form directly to the attention of the City Manager.  Instructions are provided on the form.
